INDIAN EDUCATION

PROGRAMS:

Collinsville Public Schools offer two federal programs for Native American Students. Title VI and Johnson O’Malley (JOM) Indian Education Programs. For both programs we need a 506 Form filled out one time with black ink and kept on file at the Indian Ed. Office. This form can be found online at collinsville.k12.ok.us under Explore,-Departments-Indian Education-then click on the plus sign. It requires a date and the signature needs to be physically written, it can not be electronically signed. A copy of the students CDIB or Tribal Membership Card should be submitted along with the 506 Form. If the student does not have citizenship through a tribe, we can apply them for the program with the parent or grandparent citizenship card along with birth certificates to show lineage.

The Title VI Program offers tutoring in PK-5th grades during school hours. Grades 6-8th grades are offered tutoring during the school day when notices are sent out to individual students. 6-12th grades are offered after-school tutoring at the middle school library on Tuesday and Thursday. Students need to bring their Chromebooks.

The JOM Program offers basic school supplies to PK-5th grades at the beginning of school. 6th-12th grade will be supplemented with paying for half of the Chromebook Protection Plan. K-5th grades receive an allotted amount for the Spring Book Fair. Students in 10-12th grades are reimbursed $50 for one ACT/SAT Test per school year. Students need to submit copies of the online receipt signed by them with the correct mailing address to kerryeubank@collinsville.k12.ok.us.

JOM also pays for $40 of the Senior Graduation Cap and Gown. The JOM pays for membership fees (i.e., Oklahoma Indian Student Honor Society (OISHS), NIEA, Cherokee Youth Council, etc.) up to a certain amount.

HISTORY/LANGUAGE CLASSES:

Cherokee History/Language Classes for JOM Students in grades 1-5th grades are held after school in their respective buildings on Tuesdays and Thursdays. Classes for 6-12th grades are also held after school at the middle school on Monday.
